Emergency Certificate (EC)

  1. Application form for issuance of Emergency Certificate (EC) can be filled online at the following link:https://portal5.passportindia.gov.in.
  2. Self-attested copies of passport's first & last pages including all the pages of observation (if any), made by the Passport Issuing Authority.
  3. A copy of passport page containing visa/work permit/permanent residence status;
  4. Four passport sized photographs (coloured);
  5. An Affidavit stating how and where the Passport got lost orstolen/damaged (Annexure "L");
  6. Copy of police report including case no. stating the circumstances in which passport is lost (in case of lost passport);
  7. A copy of valid air-ticket to return to India;
  8. Proof of Residential address in South Africa;
  9. Duly filled Personal Particular Form;
  10. Any other document as specified by the Consular Officer;
  11. Fee (via EFT): ZAR 254/- (including ICWF Fee of Rand 30.00)/ZAR 262 only (via Card) is charged for issuance of an EC.

Note: It may be noted that issue of Emergency Certificate is intended for one way travel to India only and results in cancellation of existing passport of the holder. On arrival in India, he/she will have to contact the concerned Passport Issuing Authority in India for issue of new passport.
